Requirements:

Minimum of one-year experience working with school-age children with diverse backgrounds. Curriculum and activity development experience preferred.Bilingual (Spanish/English) preferred – if applying as bilingual candidate, must speak and understand Spanish proficiently, including conversational Spanish.Knowledge of community resources for adults and children needing emergency services including legal, financial support, housing and/or basic needs. Assist Homeless Prevention Rapid-Re-Housing Project (HPRP) DESCRIPTION: clients transition to stability through case management. Provide case management to HPRP clients. Assess needs, provide referral information, and connect clients to community resources. Initiate and conduct educational sessions. Assist in planning phases that will result in client empowerment. Provide information and emotional support in a non-judgmental, non-directive manner to clients. Maintain accurate records and appropriate documentation of services.

Attend Sheltering Arms Senior Service’s CEAP training and other meetings as required. Provide outreach communication to qualifying individuals. Conduct initial screening of applicants and schedule appointments Perform all intake duties and determine applicant eligibility for the appropriate component under the Comprehensive Energy Assistance Program. Perform data entry, and scanning of documents. Communicate to all staff and volunteers HIPPA guidelines. Ensure all geographic areas within the allocated zip code zones are adequately covered throughout the program. Conduct enrollment drives on- and off-site. Arrange home visits to homebound clients. Provide status of application to inquiring clients. Complete more than the number of applications per quarter as required in Interagency Agreement with Sheltering Arms Senior Services. Ensure all applications are accurate and complete to ensure reimbursement. Maintain applicant files, records and tracking system ensuring client confidentiality. Complete and submit monthly reports to comply with deadlines.
Requirements:
Associate’s degree (A.A.) or equivalent from two-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. Experience working in a social service setting required. Experience working with elderly population preferred. Must have good organizational skills, detail oriented, and proficient computer skills (Microsoft Word, Excel, database and Outlook). Bilingual Spanish/English preferred. HPRP Caseworker

Provide information on the HPRP Program to families referred. Interview clients and gather data to determine eligibility status of families referred to the HPRP Program and the appropriateness of serving the referred families with HPRP funds, including determining whether or not the potential participant would be homeless except for the assistance provided by HPRP. Determine the amount of assistance to be provided. Work with families to meet their housing needs and help them to obtain housing stability. Develop an individualized housing service plan, including a path to permanent housing stability subsequent to HPRP financial assistance. The plan should include short- and long-term goals and conditions that must be met in order for future assistance to be provided. Coordinate client referrals, services and linkages to resources provided by Interfaith Caring Ministries and resources provided by outside contractors, collaborating partners and others for participants and their children. These may include but are not limited to basic needs assistance (food, clothing) (not funded by HPRP funds), employment services (not funded by HPRP funds), legal assistance to remain in their homes, financial literacy (budgeting, credit counseling), housing search and placement, etc. Serve as a liaison for referral agencies.. Assist clients in enrolling for public benefits. Advocate on behalf of clients to access appropriate services.Maintain contact with client to monitor progress.
Requirements:
Minimum of 3 years of experience providing case management to homeless persons and/or persons at risk of becoming homeless. Experience with government funded housing programs is a plus. Experience with non profit/government accounting preferred. Excellent written and oral communication skills; Effectiveness in motivating people to accomplish a goal or task; Self-starter; Excellent computer and reporting skills.Bachelors.
